We can't wait for your dancer to experience summer camp at Long Beach's most LOVED dance studio, voted by our community!

We are so excited for our 2024 Summer Camp and Dance Intensive! Dancers will enjoy a plethora of dance, arts and activities. You can register by leaving a $100 deposit by June 1st, with the $150 balance due June 24th when camp starts. Full camp fee of $325 is due for registrations thereafter.

CONSENT AND ACKNOWLEDGEMENT

I hereby give my approval for my child’s participation in any and all activities prepared by Gravity Dance Companyduring the selected camp. In exchange for the acceptance of my child’s participation by Gravity Dance Company, I assume all risks and hazards incidental to the conduct of these activities and release, absolve, and hold harmless Gravity Dance Company and all its officers, agents, employees, and representatives from any and all liability for injuries to my child arising out of travel to, participation in, or return from the selected camp sessions.

In the event of injury to my child, I hereby waive all claims against Gravity Dance Company, including its teachers, counselors, affiliates, participants, and, if applicable, the owners and lessors of premises used to conduct the event.

MEDICAL RELEASE AND AUTHORIZATION

As the parent and/or legal guardian of the above-named child, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional for the minor child in the event of a medical emergency which, in the opinion of the attending medical professional, requires immediate attention.

Permission is also granted to Gravity Dance Company, including its affiliates, teachers, counselors, and staff, to provide minor injury care or emergency medical treatment prior to the child’s admission to a medical facility, if deemed necessary.

This release is effective for the dates and duration of the registered season.

This authorization is given voluntarily and with the sole purpose of permitting medical treatment under emergency circumstances, for the protection and well-being of the named minor child in my absence.
